If you are a fertility patient, please fill out both the acupuncture intake and fertility intake forms.
Bring all EWA intake forms along with any pertinent medical tests/blood tests/reports/basal temp charts and a complete list of all medications, over-the counter meds, herbs, supplements and vitamins to your first appointment!
Also consider bringing a calendar to schedule future appointments and bring an interpreter if needed.
